Laxmi Chhetri

Founder & Director, BSN, RN-BC

Every great mission begins with a personal journey. For Laxmi Chhetri, BSN, RN-BC, the path to founding Dignity Transition Care started humbly and grew into a calling rooted in service, compassion, and dignity.

Laxmi began his healthcare career at the ground level, working as a housekeeper and clinical assistant at Ascension. Those early experiences gave him a deep appreciation for the importance of safety, cleanliness, and compassion in healing environments. They also showed him how even small acts of care can make a meaningful difference for patients and their families.

Determined to expand his ability to serve, Laxmi pursued nursing and earned his Associate Degree in Nursing from Austin Community College. He continued his education at the University of Texas at Arlington, where he is completing his Master of Science in Nursing as a Family Nurse Practitioner.

Today, as the founder of Dignity Transition Care, Laxmi is committed to helping families access compassionate, reliable, and personalized care that allows their loved ones to remain safe, supported, and respected at home.

Will Maddux

Case Manager, RN

Will Maddux is a dedicated Registered Nurse with 12 years of nursing experience and a strong passion for providing compassionate, patient-centered care. He has worked in a variety of healthcare settings and is committed to helping patients feel comfortable, informed, and supported throughout their care journey.

Known for his calm and caring demeanor, Will takes pride in building trusting relationships with patients and their families. He believes excellent healthcare begins with listening, empathy, education, and a genuine commitment to each person’s well-being.

When he is not caring for patients, Will enjoys staying active and spending time with his wife and daughter. He is proud to be part of Dignity Transition Care and looks forward to helping each patient achieve the best possible quality of life.

Huy Huynh

Case Manager, RN

Huy Huynh was born in Saigon, Vietnam, and came to America with a dream of building a better future through hard work and perseverance. That journey shaped his passion for helping others and ultimately led him to a career in nursing.

As a Registered Nurse at Dignity Transition Care, Huy is passionate about providing compassionate, patient-centered care that helps individuals feel safe, respected, and supported at home. He believes healthcare is about more than treating illness. It is about building trust, preserving dignity, and supporting families through difficult moments.

Huy’s dedication to exceptional care earned him the DAISY Award, a recognition that continues to inspire his work every day.

Outside of nursing, Huy enjoys working on cars, staying active, and continuing to grow personally and professionally. He is currently pursuing his goal of becoming a Nurse Practitioner in cardiovascular care so he can further serve his patients and community.

Jean-Claude Ntiranyibagira

Case Manager, RN, BSN

Jean-Claude Ntiranyibagira is originally from Burundi and immigrated to the United States in 2006. His interest in healthcare began at an early age, inspired by the positive impact medical professionals can have on individuals, families, and communities.

After moving to the United States, Jean-Claude pursued nursing and discovered an even deeper passion for patient care. He values the close connection nurses build with patients and the opportunity to help, educate, and promote better health in both home and clinical settings.

Jean-Claude holds a Bachelor of Science in Nursing and is also certified in cardiovascular nursing. He values integrity and believes every patient deserves respectful, high-quality care wherever they are receiving support.

He plans to pursue a Nurse Practitioner degree in geriatrics to continue growing his expertise and better serve older adults and their families.

Dr. Saleem Khawaja

Volunteer, MD

Dr. Saleem Khawaja is a board-certified internal medicine physician with more than seven years of experience caring for patients in the hospital setting. Throughout his career, he has supported individuals and families through complex medical conditions, care transitions, and discharge planning.

His passion for home care became personal when he sought support for a loved one and saw firsthand how compassionate caregiving can improve recovery, independence, and quality of life. That experience inspired his involvement with Dignity Transition Care and strengthened his commitment to helping families feel supported during important transitions.

Dr. Khawaja is dedicated to serving families in the Austin community by promoting high-quality, compassionate, and affordable care. He believes that when skilled nursing oversight is combined with dedicated caregivers, families gain the confidence, guidance, and support they need to help their loved ones remain safe and comfortable at home.
