Our Services
Dignity Transition Care provides compassionate, high-quality transitional Non-Medical care that ensures patients recover safely and comfortably at home. We aim to prevent hospital readmission rates, reduce the length of hospital stays, prevent surgical complications, improve medication adherence, enhance outpatient services, and promote independence while supporting families throughout every step of the healing process. Our services are not limited to those listed below.
Services We Provide
Personal care services help individuals with difficulty managing basic activities due to age, injury, or disability. These services include:
Assistance with Bathing and Grooming: Many individuals require help bathing, showering, and maintaining personal hygiene. Caregivers ensure that the person stays clean and feels comfortable while preserving privacy and dignity.
Dressing and Undressing: Some clients may need help getting dressed or choosing appropriate clothing. Caregivers can assist with putting on clothes and any fastenings or adjustments that may be difficult due to mobility or physical impairments.
Toileting Assistance: For clients who struggle with getting to or using the bathroom independently, caregivers provide discreet and respectful assistance. This could involve helping the client get to the toilet and supporting them with incontinence care, if necessary.
Mobility Support: Caregivers can assist clients with transferring from one location to another, such as getting out of bed, moving from a chair to a wheelchair, or walking around the home. This service is crucial for those with limited mobility or after surgery.
Loneliness and isolation can have adverse effects on mental and emotional health, especially for seniors. Non-medical home care helps combat this by offering:
Companionship: Caregivers provide friendly, meaningful conversation and emotional support, which can help combat feelings of loneliness. Companionship services often include chatting about the person’s life, reminiscing, or simply spending time together to provide comfort.
Shared Activities: Caregivers can participate in activities that the client enjoys, such as reading, watching TV, doing crafts, or playing games like cards, puzzles, or board games. This can stimulate mental activity and enhance the individual’s overall well-being.
Social Outings: For those who enjoy getting out but may need assistance, caregivers can accompany the client to appointments, social events, parks, or even just for a walk in the neighborhood, helping maintain connections to the outside world.
Mental Stimulation: Caregivers may help stimulate mental engagement by introducing new hobbies, puzzles, or light exercises to keep the mind sharp and prevent cognitive decline.
Proper nutrition is key to maintaining health, and non-medical home care services assist in this area by:
Meal Planning: Caregivers can help plan meals that meet the client’s dietary needs, preferences, and restrictions. They may also work with families to ensure meals are tailored to special conditions, such as low-sodium or diabetic diets, but again, these are purely non-medical considerations.
Cooking and Meal Prep: Caregivers can prepare meals from scratch or reheat pre-prepared meals. This ensures the client is getting nutritious, well-balanced meals without the stress of cooking for themselves.
Grocery Shopping: If clients cannot get to the store, caregivers can assist by shopping for groceries or helping order food online, ensuring the client always has access to fresh ingredients. Caregivers may also check labels to help clients avoid foods that don’t align with their dietary preferences.
Meal Assistance: Some clients may struggle with feeding themselves due to physical limitations. Caregivers assist with eating when necessary, helping cut food into smaller pieces or providing the proper utensils to make the process easier.
A clean and organized environment can significantly improve a person’s quality of life, and non-medical caregivers help with tasks that maintain a safe and pleasant home environment:
Household Cleaning: Caregivers help with light cleaning tasks, such as dusting, vacuuming, mopping floors, and wiping down surfaces. They ensure the home remains tidy and hygienic, making it safer and more comfortable for the client.
Laundry: Non-medical caregivers can assist with washing, folding, and organizing clothes, towels, and bedding. This helps prevent the buildup of laundry that may become overwhelming for individuals with limited mobility or stamina.
Dishwashing and Kitchen Care: After meals, caregivers can clean dishes, wipe down counters, and ensure that the kitchen stays orderly. Keeping the kitchen environment clean reduces the risk of accidents, such as slipping or tripping.
General Organization: Caregivers can help clients organize personal belongings, put away groceries, or keep areas like closets or pantries neat and easily accessible.
Non-medical home care services often include providing transportation and assisting with running errands, which are crucial for clients who cannot drive themselves.
Accompanying Appointments: Caregivers can drive clients to medical appointments, social events, or religious services. While they won’t provide medical care during these visits, they ensure the individual gets to their appointments safely and on time.
Grocery Shopping and Errands: Besides medical visits, caregivers can run errands like grocery shopping, picking up prescriptions, or going to the post office. This allows clients to continue participating in daily activities without relying on others.
Social Engagement: Some clients may want to visit friends or family, or enjoy outings like a park, library, or coffee shop. Caregivers provide transportation and companionship during these trips, ensuring the client’s social life remains active.
Family caregivers often need time to recharge. Non-medical home care provides relief by stepping in when the primary caregiver needs a break:
Temporary Care: Family members can take a vacation, attend to their needs, or rest without worrying about their loved one’s care. Caregivers provide the same high level of support, ensuring consistency and quality in the care received.
Flexible Schedules: Respite care can be arranged temporarily, whether for a few hours, a full day, or an extended period. This flexibility allows family caregivers to focus on their well-being.
Peace of Mind: Knowing that a qualified caregiver is present can give family members peace of mind while they’re away, ensuring their loved one is safe, comfortable, and well cared for.
Non-medical caregivers also provide safety-related services, particularly for individuals with memory issues, mobility problems, or cognitive impairments such as dementia. These services include:
Fall Prevention: Caregivers assist with mobility and ensure the living environment is safe by removing potential tripping hazards like loose rugs, clutter, or poor lighting.
Supervision and Monitoring: Caregivers can monitor individuals prone to wandering or engaging in risky behaviors. This is particularly important for individuals with Alzheimer’s or dementia, who may forget where they are or become confused about their surroundings.
Emergency Preparedness: While non-medical caregivers aren’t medical professionals, they are trained to recognize signs of distress, such as confusion, disorientation, or falls, and to take appropriate action, like calling family members or emergency services when necessary.
If services include the use of the client’s funds:
Suppose the services involve using the client’s funds. In that case, the Agency will obtain authorization from the client or the responsible party before providing grocery shopping, bill-paying assistance, or any other activities that require access to or use of the client’s funds.
